A DE-964 Certificate (Driver's Ed Certificate given to students by a certified Driver's Education School)
A Certified Birth Certificate (With a state seal or ink stamp, absolutely no photo copies. DPS wants to see the seal or ink stamp to verify that the Birth Certificate is an original.)
A Social Security Card (The actual card, again no copies will be accepted.)
TEA/VOE form (This form is a verification of enrollment in high school and can only be received from your child's high school. The form is based solely upon attendance and is only valid for one month.)
